Introducing Fatal Perils and New Wounds System

New revisions to wounds make them more deadly. Each wound you accrue now confers a -1 penalty to all actions you take. This is reflected in the Hero Creator. Mighty is no longer reduced when you take wounds. There is now a new status to describe having 0 HP called vulnerable, which just means you accrue wounds when at 0 HP.
As a corollary to this, there is now a third fatal peril. Fatal perils represent sources of harm that can render you unconscious, on death's door, or dead immediately. This covers situations where the damage is extreme, such as in falling or crushing damage, called shots to vital areas, or when there is a hostage situation (e.g., slitting a grappled character's throat).
